To
use the application, click on the Employment
Application link and open the form in your browser
window. The application is a PDF form that you
can fill out by typing the information into the
fields. Once you have filled out the required
information, click on the "Email" link on the
toolbar and your email client will open. Send
the form to
christy.orr@triadhospitals.com. You should
also click on the "Save a Copy" button on the tool
bar and save a copy to your desktop. You can
also print the application and mail or bring it
with your resume to
