In order to expedite your visit to Woodland Heights Medical Center, we are offering several of our forms on-line. These forms are in Adobe PDF format and you will need to install the free Adobe Reader to download and print the forms. You can fill out the forms at home and bring them with you when you visit WHMC. To use the application, click on the Employment Application link and open the form in your browser window. The application is a PDF form that you can fill out by typing the information into the fields. Once you have filled out the required information, click on the "Email" link on the toolbar and your email client will open. Send the form to jo.bentley@woodlandheights.net. You should also click on the "Save a Copy" button on the tool bar and save a copy to your desktop.
